A student is eligible for homebound services if:
The student is unable to access his or her school campus due to a known illness and/or hospitalization or due to a physical or mental impairment or condition that requires recoupment at home.
Students who have health impairments due to chronic or acute health problems (e.g, heart condition, tuberculosis, rheumatic fever, nephritis, asthmatic sickle cell anemia, hemophilia, epilepsy, lead poisoning, leukemia, or diabetes), which are severe enough to limit strength, vitality, or alertness which adversely affect educational performance can be eligible for homebound services.
Verification from the student's physician, psychiatrist or other relevant professional and the student's parents or legal guardian is necessary in determining eligibility.
